We understand that transitioning to Supported Living can be challenging for everyone involved. That's why we encourage you to develop relationships with key workers and follow a well-defined plan towards a sustainable outcome. Families can play a crucial role in the process by spending time at the project and actively engaging with staff members to create a person-centered support plan. Our transition goals are outcome-oriented, focusing on your strengths, areas of need, and living skills, including education and employment.
Come and Visit Us
We place great importance on building strong relationships with you, your family, and external professionals from the very beginning. To facilitate this, we encourage potential individuals to visit us early on. These visits help us get to know each other better, lay a solid foundation, and allow key workers to establish a productive relationship before admission.
